The Latino Health for All Coalition recognized a particular gap in quality health services related to diabetes that affected many members of the Latino community.

Lead Coalition partners in this project were the JUNTOS/Center for Advancing Latino Health, Community Health Council, and Riverview Health Services.

The lead partners, with input from the Health Access Committee, developed and refined the playbook for implementation with partner health organizations in Wyandotte County. They provided two forms of support to participating health organizations: a) Brief informational session to present a case for diabetes prevention and b) Organizational support to implement client referrals to a diabetes prevention program.

Some Accomplishments:

  • Provided DPP screening and education at local health fairs
  • Supported diabetes prevention classes for Spanish-speaking participants
  • Many participants reached their 5% weight-loss goal, a 58% decrease in risk for Diabetes
